Overview
The goal of transition care is health optimization and maximizing the life potential of youth. However, there is significant evidence that most young people are not successfully navigating this transfer of care. Barriers to effective transition occur throughout the process. The curriculum for this symposium aims to provide practitioners with an update about new activities and projects geared towards improving pediatric to adult transitions. In addition, aspects of this curriculum were created to provide practitioners with the most up-to-date knowledge about health care guidelines and standards necessary to deliver effective patient care to those patients who require transitional care.

WHO SHOULD ATTEND
This activity is intended for family practitioners, general practitioners, general internists, pediatricians, multiple specialties, nurses, nurse practitioners, physician assistants and social workers.

Objectives
  1. Create a collaborative environment to provide opportunity to share best practices.
  2. Identify health care transition efforts in order to synergize transitional care across Maryland.
  3. Arrange an opportunity to share current work and celebrate collective successes and challenges.

Accreditation
This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Johns Hopkins University School of Medicine and J-Train: The Johns Hopkins Transition Independence Network. The Johns Hopkins University School of Medicine is accredited by the ACCME to provide continuing medical education for physicians.

CREDIT DESIGNATION STATEMENT
The Johns Hopkins University School of Medicine designates this live activity for a maximum of 3.50 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

MAINTENANCE OF CERTIFICATION
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 3.50 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

American Board of Pediatrics (ABP): Completion of this activity and associated assessment enables the participant to earn 3.50 ABP MOC Part 2 points.

OTHER CREDIT
Continuing Medical Education credit is also available from other applicable medical specialty organizations.

American Nurses Credentialing Center (ANCC)
accepts AMA PRA Category 1 Credits™ from organizations accredited by the ACCME.

American Academy of Nurse Practitioners National Certification Program accepts AMA PRA Category 1 Credit™ from organizations accredited by the ACCME.

The Maryland Board of Social Work Examiners certifies that this program meets the criteria for 3.50 credit hours of Category 1 or .3 CEUs of continuing education for social workers and associates licensed in Maryland. The Johns Hopkins University School of Medicine is an approved sponsor of the Maryland Board of Social Work Examiners for continuing education credits for licensed social workers in Maryland.
