Overview
This half-day virtual conference acknowledges the alarming increased incidence of Diabetes in Bermuda with a multi specialist consideration towards the diabetic patient. This should result in improved patient outcomes through an agreed treatment plan as a result of the specialist approach. Integration being a focus of care within chronic diseases management which will streamline pathways with reduced duplication in services. In tandem, improving coordination of care, educational opportunities and mental health well-being. This approach to this patient care results in enhanced patient satisfaction and decreases the period from diagnosis to treatment. The multidisciplinary approach improves collaboration leading to reduced communication and care errors, ensuring the patient receives the customized care that they need.

Objectives
  1. Educate physicians and other clinical professionals on the benefits of a multidisciplinary approach to optimize patient outcomes providing real-life guidance, coaching and support.
  2. Utilize evidence-based care, the multidisciplinary specialist team are responsible for the provision of services and treatment in which they are experts.
  3. Recognize the effectiveness of receiving care according to the clinical practice guidelines, including psychosocial support and understanding the impact of living with a chronic disease upon an individual’s overall wellbeing
