The ever changing needs of patients, ongoing advancements in clinical knowledge, and evolving practice environments create gaps that must be addressed across a broad range of topics. Provider time is being stretch too thin, causing many providers to spend two to three hours after the end of their work day just to keep up. This is causing burnout across the healthcare system and specifically in primary care at JHCP, in addition to destabilizing the work-life balance in our physicians.

1. Outline and adhere to the new updates to regulations practices for primary and ambulatory care topics presented in this series.
2. Outline and apply new updates to best practices for primary and ambulatory value-based care topics presented in this series.
3. Describe the application of Evidenced Based Practice (EBP) in primary and ambulatory care for the topics presented in this series.
4. Formulate inter-professional collaboration and education to support Team-Based Practice in primary and ambulatory care based on what was presented in this series.
