WHO SHOULD ATTEND
Anyone in the health care industry interested in safely improving health care value, including medical providers of all specialties (doctors, nurse practitioners, nurses, physician assistants, pharmacists, house staff, medical and MPH students), including hospital leadership (CEO, CQO,CMO, CFO), performance improvement companies, regulatory leads and insurance administrators. Health care performance improvement, clinical decision support and education companies are invited to participate as exhibitors and sponsors.

DESCRIPTION
As the cost of health care in the United States continues to rise, of utmost concern are reports that medical bills are creating substantial hardships for patients and that the cost of health care is now the biggest financial concern facing American families. Health care debt is a leading driver of personal bankruptcy and home foreclosure. Both insured and uninsured patients are burdened by medical bills. Confounding these issues is evidence that patients avoid necessary care (prescription refills, doctors appointments) because of cost. In response to these challenges, providers across the country are reflecting on their practice to identify opportunities and implement initiatives that improve patient care quality and safety while reducing costs .

The Architecture of High Value Healthcare National Conference is designed to be a platform to efficiently advance high value care performance improvement across the country. The mission of the annual conference is to share leading practices and provide immersive skill-building experience around value-based care. Multiple plenary abstract presentation sessions are designed to broadly advance initiatives that have improved health care quality and safety while reducing costs of care for patients, investigate outcomes to ensure that practice refinement maintains or improves diagnostic or therapeutic efficacy, and determine the best methodologies to improve the value of care providers deliver. Performance improvement guidance and support are provided through multiple focused workshops and round-table information sharing sessions monitored by national experts.  Details are provided on our website: www.hvpaa.org/conference.

LATE FEE AND REFUND POLICY
A $50 late fee applies to registrations received after 5:00 p.m. ET on July 19, 2019. A handling fee of $50 will be deducted for cancellation. An additional fee may apply for cancellation of other events, including workshops and social activities. Refund requests must be received by fax or mail by July 19, 2019. No refunds will be made thereafter. Transfer of registration to another Johns Hopkins activity in lieu of cancellation is not possible. 


Accreditation

The Johns Hopkins University School of Medicine is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians. 

CREDIT DESIGNATION STATEMENT 
The Johns Hopkins University School of Medicine designates this live activity for a maximum of 18.25 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

MOC STATEMENT

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 18.25 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity.
